Diltiazem Hydrochloride (ZEMDIL) 30mg Tablet, a calcium channel blocker used primarily for the treatment of cardiovascular conditions. It is available in a 30mg film-coated tablet form, packaged in bottles containing 100 tablets. Diltiazem Hydrochloride (ZEMDIL) 30mg Tablet works by relaxing the blood vessels, reducing the heart’s workload, and improving blood flow, making it effective for treating conditions like hypertension and angina. It can also be used for certain types of arrhythmias (irregular heartbeats).

Indications:

Diltiazem Hydrochloride (ZEMDIL) 30mg Tablet is prescribed for the following conditions:

• Hypertension (High Blood Pressure):
  • Used to lower blood pressure, helping reduce the risk of strokes, heart attacks, and kidney issues.
• Angina Pectoris (Chest Pain):
  • Relieves chest pain by improving blood flow to the heart.
• Supraventricular Tachycardia (SVT):
  • Used in the management of certain types of arrhythmias, especially supraventricular tachycardia, a condition characterized by an abnormally fast heart rate originating from the upper chambers of the heart.
• Atrial Fibrillation or Flutter:
  • May be used to control the heart rate in patients with atrial fibrillation or atrial flutter.

 

Dosage and Administration of Diltiazem Hydrochloride (ZEMDIL) 30mg Tablet:

For Hypertension:

• Initial dose: 30 mg once or twice daily.
• Maintenance dose: Typically 30 mg to 120 mg daily, depending on individual response.

For Angina:

• Initial dose: 30 mg to 60 mg 3 times daily.
• Maintenance dose: May be adjusted up to 360 mg daily, based on clinical response.

For Supraventricular Tachycardia (SVT):

• Typical dose: 30 mg to 60 mg 3 times daily.

Administration:

• Oral use: Take the tablet with a glass of water. It is recommended to take it with food to reduce potential stomach irritation.
• Do not crush or chew the tablets. Swallow them whole.

Side Effects:

While Diltiazem Hydrochloride (ZEMDIL) 30mg Tablet is generally well-tolerated, some individuals may experience side effects. Common and rare side effects include:

Common Side Effects:

• Dizziness or lightheadedness.
• Headache.
• Flushing (redness or warmth of the skin).
• Fatigue or tiredness.
• Swelling (edema), especially in the ankles or legs.
• Bradycardia (slow heart rate).
• Constipation.

Serious Side Effects (Less common, but may occur):

Contraindications:

Diltiazem Hydrochloride (ZEMDIL) 30mg Tablet should not be used in the following conditions:

• Severe Bradycardia (extremely slow heart rate).
• Second- or Third-Degree Heart Block (without a pacemaker).
• Severe Hypotension (low blood pressure).
• Sick Sinus Syndrome (a condition where the heart’s natural pacemaker malfunctions).
• Acute Myocardial Infarction (Heart Attack) with associated low blood pressure.
• Severe Liver Impairment.
• Known Hypersensitivity to Diltiazem or any component of the formulation.

Precautions for Diltiazem Hydrochloride (ZEMDIL) 30mg Tablet:

• Heart Failure:
  • Caution is advised when using Diltiazem in patients with heart failure, as it can worsen the condition in some individuals, particularly if used in high doses.
• Renal or Hepatic Impairment:
  • Diltiazem is metabolized in the liver, so individuals with liver disease should use this medication with caution and may require dose adjustments.
  • Patients with kidney disease should also be monitored closely, as the drug’s effects can be altered.
• Hypotension:
  • Monitoring is required in patients with low blood pressure or those prone to hypotension, as Diltiazem can cause a further drop in blood pressure.
• Diabetes:
  • Diltiazem may mask the symptoms of hypoglycemia (low blood sugar) in diabetic patients.
• Pregnancy and Breastfeeding:
  • Pregnancy Category C: Diltiazem should be used during pregnancy only if the potential benefits justify the potential risks to the fetus.
  • It is not recommended during breastfeeding, as it is excreted in breast milk.

Drug Interactions for Diltiazem Hydrochloride (ZEMDIL) 30mg Tablet:

• Beta-blockers: Combining Diltiazem with beta-blockers can result in severe bradycardia or heart block.
• Digoxin: Co-administration of Diltiazem with digoxin (a heart medication) may increase the risk of bradycardia.

• CYP3A4 Inducers (e.g., rifampin, carbamazepine): May decrease the effectiveness of Diltiazem by lowering its blood concentration.
• Alcohol: Drinking alcohol while taking Diltiazem can increase the risk of low blood pressure and dizziness.

Overdose for Diltiazem Hydrochloride (ZEMDIL) 30mg Tablet:

Signs of overdose may include:

• Severe bradycardia (slow heart rate).
• Hypotension (low blood pressure).
• Dizziness, fainting, or lightheadedness.
• Heart block (difficulty with electrical impulses in the heart).

Management:

• Atropine may be administered to reverse bradycardia.
• Calcium salts (such as calcium gluconate) may be used in severe cases of overdose to reverse the effects.
